Practice Connect: EHR Reporting & Recall
Independent practices sit on years of clinical data they can't easily use — the chart is built for the patient in front of you, not for the question “who is overdue?” Practice Connect reads a practice's record through Practice Fusion's official FHIR API and turns it into the lists and reports staff would otherwise assemble by hand. It is read-only by design: we never write to the chart, and the EHR stays the source of truth.

Frequently asked questions
- Can Practice Connect change anything in our chart?
- No. Practice Fusion's FHIR API is read-only, and Practice Connect is built the same way — it reads the data a practice authorizes and writes nothing back. Your EHR remains the source of truth for the clinical record.
- Do you sign a Business Associate Agreement?
- Yes. R21 signs a BAA with the practice before any patient data moves. The practice is the Covered Entity and R21 is the Business Associate, which means the practice keeps control of the data and of how it is used.
- What patient data does it actually read?
- Only the categories the practice authorizes — typically demographics, encounters, conditions, medications, allergies, lab results, immunizations, and related clinical documents. Every access is written to an audit log recording who read what and when.
- Is our patient data used for marketing or to train AI?
- No to both. Patient data is never sold, never used for advertising, and never used to train artificial intelligence models. Recall and outreach lists belong to the practice and are acted on by the practice under its own patient relationships.
